This picture was based on a brilliant Bleach headcanon from RomaniaBlack (see her here:

[link]) on Squad 4 captain Retsu Unohana being somewhat of a Kenpachi-like person: violent, bloodthirsty and gets "thrills" from fighting to the death (Yeah, anyone picking up that innuendo yet?) but eventually settling down and becoming the Unohana we all know and love (and fear greatly) Hell, this headcanon has been on the minds of other Bleach fans:

So, anyway, I kinda agree with this headcanon as it explains (to me, at least) why EVERYONE (Especially Squad 11, even though they're still stupid enough to hate and mock them when she's not around) fears her and I was inspired to make this artwork.
